The book title and author name boxes are at the top, giving plenty space to record the important information.

Underneath that, there is a block with the fun question words we will be using.

For this worksheet, I focus on our six questions.

Who is the main character? Include any information you want to give. Name, age, character traits...

What does the main character want? I'm thinking along the lines of the plot of the book. What does the main character want to have happen? World peace? Getting an A in geometry? Finding the magical map to a secret treasure that saves their hometown?

When does the book take place? Is it in the present, in the past, or in the future? Is it on a weekend or throughout a month or a year?

Where does the book take place? So many places, real and fiction. Is it at the characters' homes, or in a spooky forest? Maybe it's throughout the entire world!

Why does the main character act the way they do? Or another way, what is their motivation?

How does the main character achieve their goal?

Now that the questions are done, here comes the summary and, lastly, the five-star rating space.

There you have it! The Who, What, When, Where, Why & How book worksheet!

Happy Reading!
