Reading For Fun: Book Survey

Readers find books in different ways. They decided to read a book for different reasons. They actually spend their reading time in different places. I thought it would be fun to find a way to capture these moments...


...and so a new worksheet is born!


I'm calling this worksheet: Book Survey. It brings out the "story" of why a book was read, not just about the book's actual story. Don't worry, there is still a space for a book summary and five-star rating!

Book Survey


Book Survey-Landscape


A survey has to have two main components—questions and answers. I thought of three questions I thought would be fun to keep track of how a book came to be read.


The first question is: How did you find out about the book? The answers provided, which can be checked with a box next to each, are: a friend, a teacher, a librarian, my parents, I found it myself, or other. The other option has a line where a new answer can be written.


The second fun question I came up with is: Where did you spend the most time reading? Prompted answers are: home on the couch, school, the library, in my room, can't say it's a secret, and other—also with a line to add their own answer. The secret option is just for fun. Maybe a secret tree-house, or comfy book fort! Who knows!


The third question is: Why did you want to read this book? Answers include: the title, the cover, the back of the book, I heard it was good, the author, and other. I have a feeling quite a few other options will be checked with "for school" written in. But that's not a bad thing at all!


I've also included a space for the book's title and author to be recorded. As well as a summary section of the book itself and a five-star rating. All the necessary tidbits to keep a book worksheet organized.


And there you have it! A great way to capture the way a book is found and read, as well as a summary and five-star rating!
